Colorado allows the yearly reassessment and support-plan meeting on one day
Colorado HCBS members can now have the yearly reassessment visit and support-plan meeting on the same day, if they prefer. Effective September 23, 2026.
Each year, Colorado Home and Community-Based Services (HCBS) members have a reassessment visit and a support-plan meeting. Until now, those had to be on different days. Starting September 23, 2026, they can be on the same day if you prefer.
What's changing
The Colorado Department of Health Care Policy and Financing (HCPF) put out Operational Memo OM 26-062. It is in effect from September 23, 2026 through September 23, 2028.
It lets two yearly activities happen on the same day, when the member prefers it:
- The Level of Care (LOC) Screen Reassessment visit.
- The Continued Stay Review Person-Centered Support Plan meeting. This is the yearly support-plan meeting.
The memo says:
"the reassessment visit and the annual support-plan meeting (the Continued Stay Review Person-Centered Support Plan) are now allowed to happen ON THE SAME DAY when the member prefers it"
That is from HCPF Operational Memo OM 26-062.
Before this memo, case management agencies had to hold them separately. They are still two separate activities, each with its own requirements. How often they happen and what they cover did not change.
Who it affects (and who it doesn't)
This affects Colorado HCBS members, and their families, who go through the yearly reassessment and support-plan cycle with a Case Management Agency.
Nothing changes for members who want to keep the two visits on separate days.
Key dates
- September 23, 2026: The same-day option takes effect. The memo runs through September 23, 2028.
What you can do
- If two visits are hard for your family to schedule, you can ask your case manager to combine them into one day.
- If separate days work better for you, you can keep them separate.
- Keep a copy of your support plan and any notes from the meeting.
