Colorado Medicaid: shorter retroactive coverage starting January 1, 2027

For Colorado Medicaid applications filed on or after January 1, 2027, coverage for past medical bills will reach back 1 or 2 months instead of 3.

· Colorado · This has been decided and has a start date.

When you apply for Colorado Medicaid, it can help pay medical bills from the months before you applied. Today that look-back is up to 3 months. For applications filed on or after January 1, 2027, it gets shorter.

What's changing

A federal law, H.R. 1, shortens Medicaid's look-back for paying older medical bills. This is often called retroactive coverage.

The Colorado Department of Health Care Policy and Financing (HCPF) lists the change on its H.R. 1 Key Changes page. It says:

"H.R. 1 shrinks that look-back for applications filed on or after January 1, 2027: up to 1 month back for adults in the expansion group, and up to 2 months back for other groups, including children and seniors"

That is from HCPF's H.R. 1 Key Changes page.

In short:

  • Today: up to 3 months back.
  • Adults in the expansion group, applying on or after January 1, 2027: up to 1 month back.
  • Other groups, including children and seniors, applying on or after January 1, 2027: up to 2 months back.

The state rule that puts this in place (MSB 26-07-06-A) was scheduled for final adoption at the October 9, 2026 meeting of the Medical Services Board (MSB).

Who it affects (and who it doesn't)

This affects people who apply for Colorado Medicaid on or after January 1, 2027 and have recent unpaid medical bills.

It does not change:

  • Applications filed before January 1, 2027. Those keep the 3-month look-back.
  • Ongoing coverage for people who are already Medicaid members.

Key dates

  • January 1, 2027: The shorter look-back applies to applications filed on or after this date.

What you can do

  1. If someone in your family needs Medicaid to help with recent medical bills, apply sooner rather than later. The longer look-back only applies to applications filed before January 1, 2027.
  2. Keep copies of recent medical bills and the date you apply.
  3. If you are not sure whether this applies to your family, ask your case manager or whoever is helping with your Medicaid application.
